I authorize annual membership dues of $490.00, which are payable via ACH or credit/ debit card via Stripe. My signature shows that I have reviewed and agree with the terms.
TERMS & CONDITIONS

I understand that:
1. I am not required to sign this form or make Legal Defense Fund contributions as a condition of my employment by my employer or membership in the Union.
2. I may refuse to contribute without any reprisal.
3. Only union members in good standing and executive/administrative staff who are U.S. Citizens or lawful permanent residents are eligible to contribute to FOP Legal Defense Fund.
4. The amounts on this form are set by the Fraternal Order of Police Legal Defense Fund and Lodge 702 S. California Probation. I may contribute more or less by this or some other means without fear of raver or disadvantage from the union or my employer.
5. The FOP Legal Defense Fund uses the money it receives for administrative costs, including but not limited to funding the Fraternal Order of Police, Legal Defense Fund, Lodge 702, S. California Probation. Contributions to the Legal Defense Fund are not deductible for federal income tax purposes.
6. There are no refunds. Your drop date is your renewal date.
7. This authorization shall remain in effect until revoked in writing by me.
